Project idea (in English)

The goal of this project is the realization of the main target of the program – RSO4.5. The implementation of equal access to aid and increasing the sustainability of health care system, including the primary medical care, also facilitating the transition from institutional to family and community treatment. This project is directed to increasing of health care level in Zbarazh territorial community by taking measures to ensure better access to prophylaxis, diagnostics and specialist medical care, including geriatric and palliative care, including the implementation of solutions in the field of computerization health care institutions for improving the access to services in the field of health care.
